PROFESSIONAL DISCLAIMER
1. Scope of Services
Jordan Peterson Stephan, through Folx and Friends Counseling, provides Clinical Supervision and Professional Consultation to LSWs and LMHC-As in the State of Indiana.
Clinical Supervision: Formal oversight for those pursuing LCSW or LMHC licensure.
Professional Consultation: Case-specific or practice-management guidance for licensed peers.
Educational Nature: These services are professional development and do not constitute a therapist-client relationship with the Supervisee/Consultee or their clients.
2. Indiana Board Compliance & Responsibility
Credential Verification: For LMHC-As, Jordan Peterson Stephan serves as an "Equivalent Supervisor." It is the Supervisee’s sole responsibility to verify with the Indiana Behavioral Health and Human Services Licensing Board that this supervisory arrangement meets the requirements of their specific employment site and licensure track.
Documentation: The Supervisee is responsible for maintaining accurate clinical logs. Supervision forms (Form I for LSWs; Form S2 for LMHC-As) will be signed only upon the successful, ethical completion of required hours and a review of clinical competency.
Virtual Supervision: In accordance with IC 25-23.6-5-3.5, virtual supervision is conducted via synchronous, HIPAA-compliant video/audio platforms. Telephone calls, emails, and text messages do not qualify as "supervision hours" per Indiana law.
3. Clinical Liability & Indemnification
Primary Responsibility: The Supervisee/Consultee maintains full legal and clinical responsibility for their clients. Any guidance provided by Jordan Peterson Stephan is advisory.
Insurance: All Supervisees/Consultees must maintain their own professional liability (malpractice) insurance and provide proof of coverage upon request.
Indemnification: By engaging in these services, the practitioner agrees to indemnify and hold harmless Jordan Peterson Stephan and Folx and Friends Counseling from any claims or liabilities resulting from the practitioner's clinical decisions or treatment outcomes.
4. Privacy & Mandated Reporting
De-identification: To maintain HIPAA compliance, all case presentations must be fully de-identified. No Protected Health Information (PHI) shall be shared.
Mandated Reporting: Per IC 31-33-5, Jordan Peterson Stephan is a mandated reporter in Indiana. If a session reveals reasonable suspicion of child or elder abuse, neglect, or a clear threat of harm to self or others, a report will be made to the appropriate Indiana authorities (DCS/APS).
5. Organizational Leadership (MAOL)
